Ticket Information:

• Adult Tickets: $25 each
• Child Tickets (Ages 5-12): $13 each

Complimentary Ticket Policy:

• Hall of Fame Inductees: Receive one complimentary ticket for themselves and one guest.
• Coaches & Officials of the Year: Receive one complimentary ticket.
• Note: Please contact Mike Jay to confirm attendance or purchase additional tickets.

How to Register:

• Online: Purchase Adult Tickets or Child Tickets online via Square.
• Mail-in: Click here to download and print the registration form. Mail the completed form and payment to Mike Jay (904 Ella Ave, Tiffin, IA 52340) by November 27, 2026.

Important Reminder: This registration is for the Hall of Fame Banquet only. It is not for the Coaches Clinic. Clinic registration must be completed separately at ChampionshipProductions.com.
